The Massachusetts Medicaid Policy Institute, a program of the Blue Cross Blue Shield of Massachusetts Foundation, invites you to an event exploring the strategic and operational aspects of creating effective cross-sector partnerships between community-based social services organizations and health care providers. In recognition of the importance of social determinants of health on the overall health and well-being of populations, MassHealth’s Accountable Care Organization (ACO) model has incorporated explicit goals aimed at fostering coordination and integration of health-related social services. Through discussion, participants will identify best practices that may be translatable to the developing MassHealth ACOs.
This event, in collaboration with the Center for Health Care Strategies, will showcase two such partnerships: 1) the Henry Ford Health System and the Ruth Ellis Center working together to provide a range of social services including meals, peer supports, and short-term housing, in addition to primary and preventive care to LGBTQ youth in Detroit, Michigan; and 2) a partnership between the city of Central Falls, Rhode Island, a local community health center, Memorial Hospital, the YMCA, and the Department of Parks and Recreation aimed at creating a Neighborhood Health Station, a one-stop shop for all health needs, including related social services.

Have you ever had an operation for which you were prescribed pain medication, such as a hip or knee replacement? Do you suffer from chronic pain, such as arthritis or osteoporosis? Chances are you have been prescribed an opioid, even if you were unaware (e.g., Percocet, Vicodin, OxyContin, Fentanyl).
Whether you actively take an opioid or have had a prescription for one in the past, there is important information about which seniors need to be aware. With the Commonwealth of Massachusetts in the midst of an unprecedented public health epidemic, Middlesex District Attorney Marian Ryan has developed a safety program for senior citizens to raise awareness about the opioid crisis, specifically how it directly impacts them.
District Attorney Ryan will discuss the far-reaching effects of this problem as well as offer tips on how to manage prescription medication using the File of Life program. In addition, she will talk about ways to safeguard medication from theft; how to properly dispose of unused or expired medication; how to identify signs of drug addiction in a loved one, and get them help. Complimentary File of Life refrigerator magnets and personal wallet cards will be distributed to seniors who attend.
